Healthy Opportunities Pilots

North Carolina’s Medicaid program is undergoing the most significant change it has seen in over 40 years. In pursuit of Medicaid Transformation, the North Carolina Department of Health and Human Services (NC DHHS) has a vision:

“To improve the health of North Carolinians through an innovative, whole-person centered, and well-coordinated system of care that addresses both medical and non-medical drivers of health.”

Under NC Medicaid’s Transformation, NC DHHS will launch two to four regional Healthy Opportunities Pilots (the Pilots) to address both medical and social needs of select beneficiaries. Pilots must provide federally approved servicesin these four categories aimed at creating “healthy opportunities” for citizens:

  • Food
  • Housing
  • Transportation
  • Interpersonal Violence & Toxic Stress

The Pilots will serve a total of 50,000 Medicaid beneficiaries across the state and will cover both urban and rural communities. A Lead Pilot Entity (LPE) will serve as a backbone organization for each pilot to build and oversee a network of Human Services Organizations (HSOs). Up to $650 million in Medicaid funding will be available for pilots to cover:

  • two years of capacity building of LPEs and HSOs;
  • Pilot services during the demonstration.

Healthy Opportunities of The Piedmont

In October 2018, Dr. Betsey Tilson (State Health Director and Chief Medical Officer of NC DHHS) spoke at the Piedmont Triad Regional Council (PTRC) in Kernersville about NC Medicaid’s transition to managed care and the proposed regional pilots. Following Dr. Tilson’s presentation, providers of human services, health systems, and other stakeholders from the Piedmont Triad region agreed to collaborate to address social drivers of health and to apply to be a regional Medicaid pilot. The collaborative named their pilot proposal “Healthy Opportunities of the Piedmont” and designated PTRC as the Lead Pilot Entity. They chose to initiate pilot services in Forsyth, Guilford, and Rockingham counties. 

Since fall of 2018, PTRC has convened or participated in regular meetings with health and human services providers, healthcare leaders, and others with the following objectives:

  • to learn from and educate others about social determinants of health and their integration into healthcare;
  • to better understand the needs of area beneficiaries and service providers;
  • to understand the key roles, responsibilities, and design of the pilots;
  • to determine the planning and capacity building needs for the LPE and the HSO provider network;
  • to prepare for an effective RFP application process.
