Weaving Connections is a project to help people with thinking and learning differences or physical impairments make friends and feel more connected to the people, places, and things they care about. That could be anything from making friends, joining clubs, attending church, or getting a job or volunteering - anything that you enjoy and want to do more of in your life. We won’t share your name or any personal information unless you say it’s okay. You can talk freely with us. If you don’t want to answer a question, that’s totally okay.

If you like the help you get and feel connected in your community, we’d love to hear about it. If you don’t feel happy or connected, we want to know that too. What you tell us is private and won’t be shared with anyone—not even the people who help you.

At the end of the survey, you’ll find helpful resources and a way to sign up to see the results as they get continually updated.
