Help your patients take charge of their diabetes!
The Diabetes Age Well Services includes six sessions of evidence-based diabetes group classes and an individualization assessment and education plan co-created with a Registered Dietitian. The curriculum was developed by Stanford University. A physician's referral is required to participate as these are Part B Medicare benefits.
Our program is accredited by the American Association for Diabetes Educators and follows the 2017 National Standards for Diabetes Self-Management Education and Support.
Workshops are highly interactive, where mutual support and success build the participants' confidence in their ability to manage their health and maintain active and fulfilling lives.

- Physical Activity
- Blood Sugar Monitoring
- Medication Use
- Foot Care
- Communication and More!
